Block Party & Waterpalooza - August 14th

**IMPORTANT**

 

Tickets are required for children ages 4 and up. Please be sure to enter the names of the all children in your party, even those under the age of 4 when completing your purchase. Parents must accompany their child(ren) to this event for children in grades 5 and lower.

 

Wristbands will be delivered to the student's classroom for those with waivers submitted by August 12th. Please enter the teacher's name, if known.

 

Wristbands will be distributed at the event check-in table for everyone else.

Purchase your very own Cabana for Block Party! Each cabana will come with your own assigned shade tent, 2 beach towels and a foam cooler with ice and water bottles.		$50.00 
Children Receiving Wristbands (Ages 4+)
Enter the name of each child who has a purchased wristband.





Please enter all the names of any additional children that will be in your party and for which you are purchasing a wristband (ages 4 and up).


Add'l Children Attending (Ages 3 & Under) No Wristband Required
Please enter all the names of any additional children that will be in your party that are age 3 and under and for which you are not purchasing a wristband.

DELIVERY
Enter the teacher/classroom for all wristbands to be delivered. If not yet known, BSF will confirm once classes have been determined.


WAIVER
By completing this form, I give permission for my child(ren), including those attending the event independently (if permitted by Bethany School Foundation), or those under my supervision and listed herein, to participate in all Waterpalooza activities, including but not limited to the use of bounce houses and inflatable water slides.

I agree that each child will follow all directives of the Bethany School Foundation, staff and volunteers, and that each child is in good physical condition and has no medical impairment that might prevent him/her from participating.

I acknowledge that the activity is potentially dangerous and that by participating, each child may be exposed to the risk of personal injury.

I hereby release, indemnify, and hold harmless the Bethany School Foundation, Bethany Elementary School, Lammersville Unified School District, and their officers, directors, employees, volunteers, and agents from and against any and all claims for injury, loss, or damage arising out of or related to participation in Waterpalooza.

I further agree that neither Bethany School Foundation, staff, nor its volunteers are responsible for supervising children who attend the event without a parent or guardian, except while volunteers are actively operating or supervising a specific activity.
