Block Party Wristband

Please be sure to enter the names of the all children in your party when completing your purchase.

 

We will deliver wristbands to the student's classroom for waivers submitted by August 7th. Please enter the teacher's name, if known.

 

Wristbands will be distributed at the event check-in table for everyone else.

WAIVER
By completing this form, I give permission for the following child(ren) to participate in all Water-Palooza activities, including use of bounce houses and inflatable slides.  I agree that the child will follow all directives of the Bethany School Foundation, and that my child is in good physical condition and has no medical impairment that might prevent him/her from participating. I acknowledge that the activity is potentially dangerous and that by participating, my child will be exposed to the possibility of personal injury. I hereby indemnify the Bethany School Foundation ("BSF"), Bethany Elementary School and Lammersville Unified School District against all claims for injury, loss, or damage arising out of or in connection with the failure to comply with the safety instructions or directions provided by BSF and the volunteer staff.  **Parents must still accompany their child(ren) to this event for grades 5 and younger.
